ui: rebuild the header nav

Part 2 of 9 of the UI overhaul, stacked on #61. Each PR's diff is only its own step; review and merge in order.

The header as a floating glass bar, with the tools in one rail and a proper phone menu.

What changes

• Links carry an icon and a hint; a hover highlight slides between them and an active lamp marks the current page (Motion layoutId).
• One tools rail at a single 30px size: search (with its shortcut on wide screens), theme, notifications, bridge.
• "Launch a token" stays the primary action; the X link sits with the tools.
• Phone: the menu opens as a card over a scrim, the page stops scrolling behind it, and it closes on navigate.
• ThemeToggle, NotificationSettings and the bridge trigger are restyled to fit the rail; LivePulse's block variant becomes the menu's footer line.

Checks on this branch: lint, tsc --noEmit, npm test (843 pass, 0 fail, 10 skipped), next build.
