Build one gateway per inference call with a signerHeaders provider backed by resolveSignerSession, seeding the first exchange so we do not mint twice. Drop the outer SignerRefreshRequired rebuild — the library now refreshes the bearer itself.

@pymthouse/gateway-web@^0.4.0(pymthouse/gateway-web#23 — number may differ). Merge after that package is published;pnpm-lock.yamlis left on the currently resolved version so CI does not pick up afile:path. After 0.4.0 is on npm, runpnpm update @pymthouse/gateway-weband land the lockfile.Test plan
