The workflow claims Ollama is running at (127.0.0.1/redacted) but that address is loopback inside the isolated sandbox container (eth0 172.30.0.20/24), not the GitHub Actions runner where ollama serve actually starts (per prompt-optimization.lock.yml). The sandbox has no default gateway (ip route shows only the local /24, no default route) and host.docker.internal fails DNS resolution (SERVFAIL from the Docker embedded resolver 127.0.0.11). I tried: (1) curl to 127.0.0.1:11434 — connection refused; (2) curl to host.docker.internal:11434 — could not resolve host; (3) checked for a default gateway IP to try directly — none exists in the routing table. No Ollama binary, cache, or log files are present inside this sandbox filesystem either, confirming the server runs outside this container with no configured path in.
This is the same root cause already tracked in open PR #261 ("Fix Prompt Optimization Ollama access from sandbox", fixes #255), which proposes adding host.docker.internal to the sandbox network allowlist and binding Ollama with OLLAMA_HOST=0.0.0.0:11434. That fix is not yet merged, so this run cannot perform any evaluation, scoring, or hill-climbing. No repository changes were made (an incidental unstaged diff to SKILL.md metadata was reverted), and no pull request, review, or review comment was created, per the "no candidate qualifies" / infra-failure guidance in the task instructions.
