What happens
A small database last opened by a version before 4.2.11 (or one holding Thai, Lao, Khmer or similar text indexed by an older build) fails every open after upgrading with database disk image is malformed. The database is not corrupt: an upgrade step removes old keyword-index rows using the new text segmentation before the index is rebuilt. Every surface (hooks, MCP, CLI, reindex --fts) fails the same way, so there is no in-product way out.

- Take a small database written by an older build with CJK or Thai text and no title backfill yet.
- Open it with 4.10.11.
