Bring the MCP server back - #39

LookInside 3.0 ships the local MCP server again. The Host's MCPBridge (the generic Unix-socket inspection interface the bundled lookinside-mcp CLI talks to) is back, ported to the Swift Host: async requests instead of ReactiveObjC, LKChannel events instead of Peertalk, and the same newline-delimited JSON frames as 2.3.x, so the CLI and existing client configurations work unchanged. - The Embed MCP CLI phase bundles lookinside-mcp into Contents/Resources again; release.yml checks out the parent-pinned LookInside-MCP (mcp_ref) and build-release-from-tag.sh checks the bundled CLI. - LookinLiveDocument posts open/close notifications and LKStaticWindowController exposes an async reload, which the bridge's targets and hierarchy.refresh use. - DEBUG builds honour LOOKINSIDE_MCP_SOCKET_PATH (as the CLI does); release builds always use the default socket. - Tests: the listen socket, search query and identifier list suites; test.sh runs them. - README and docs/mcp.md are back.
